Spectrum Health is joining a pilot program to help burn survivors transition from the hospital to home.
The program called 'Journey Forward Kit' is from the Phoenix Society for Burn Survivors.
Six burn centers in the country are taking part.
The kit includes special products for the survivors, like lotions and creams, as well as local and national resources.
